Madison Township Crash Results in Fatality

Butler County Sheriff Richard K. Jones reports that on April 21, 2011, at approximately 2157 hours Deputies along with the Madison Township Fire Department and Life Squad were dispatched to the 1600 block of Middletown-Eaton Road in reference to a two vehicle injury accident.

Preliminary investigations indicate that a white 2004 Chevrolet truck, operated by Steven T. Cook (age 51), was proceeding eastbound on Middletown-Eaton Road when his vehicle veered across the centerline and struck a westbound gold 2002 Pontiac Bonneville head on. The operator of the Pontiac was pronounced dead at the scene by the Butler County Coroner’s Office. The name of the deceased has not yet been released. It is currently under investigation if alcohol was a factor.

This accident remains under investigation by the Butler County Sheriff’s Office Accident Investigation Unit and the Butler County Coroner’s Office.
